This form must be received by PAC-TE no later than September 15. Payment must reach PAC-TE no later than October 7.

YES, I understand that this form with all information completed, and our logo (in re-sizeable format and as reproducible in black/white or color) must be received by PAC-TE no later than September 15.

We will provide sponsorship for the following segment(s) of the Fall Conference. *

We will sponsor a Graduate student's conference stipend and expenses ($400), or one of the Beverage Breaks during the conference ($350) *
